The world of paediatric respiratory medicine is very small. A child can cross the world in less than 24 hours, well within the incubation period of many infectious diseases. Respiratory paediatricians need to know more than just what is common in their particular location. A sick child from Brazil can turn up in a London hospital, and a Canadian child to a healthcare facility in India, and the challenge for paediatric pulmonologists is to know about respiratory disease in children the world over. The idea for this book came from the INSPiRED meetings. This is the only truly international congress dedicated to children’s respiratory disease and brings together doctors of all levels of seniority and healthcare professionals from all over the world for scientific and clinical discussions. The case presentation sessions are always well attended and there is always lively debate. The purpose of this book is to bring together diagnostic and management problems from across the world. The format is a brief introduction, and then the progress of the child is set out as it evolved in real life, with questions for the reader to consider until the final diagnosis is reached. There are extensive illustrations of diagnostic tests including imaging and pathology. Each chapter has a succinct bibliography list. As well as reporting puzzling problems, the book also gives a taste of the constraints under which some colleagues operate. Not all tests are available everywhere, and for those in high-income settings, some of these cases are a salutary reminder of how lucky some of us are. Some of the problems do not have a really clear-cut solution, again reflecting real life, and this is why case discussions are so fascinating. Perhaps you will disagree with some diagnostic pathways and conclusions, but that is meant to be part of the fun – when we disagree, we learn and grow. We hope this book will stretch your mind and your horizons wherever you practice, and you will get as much value out of reading it as we did compiling it.
