Pain is an unpleasant experience that involves both sensory and emotional components and is associated with actual or potential tissue damage. It serves as the body’s warning system, alerting people to injury, illness, and other threats that can impact their well-being. Humans didn’t evolve the ability to feel pain as a form of cruel punishment; instead, it is a vital teacher—one that allows them to detect harm and commands them to protect themselves. Pain can be acute, coming on suddenly and being short-lived, or it can be chronic, lasting for extended periods, which often makes it harder to treat. And though it’s often distressing, pain also encourages healing.
The International Association for the Study of Pain defines pain as “an unpleasant sensory and emotional experience associated with, or resembling that associated with, actual or potential tissue damage.” Importantly, it’s a subjective phenomenon that has both physiological and psychological components. Pain is different from a process called nociception, which is the detection of harmful stimuli by the peripheral nervous system. In contrast, pain is the conscious interpretation of those nociceptive signals, processed in the brain.
Nociception involves specialized sensory neurons called nociceptors, which have evolved to detect potentially harmful stimuli such as extreme heat, cold, pressure, or chemical irritants. These neurons are located in the skin, muscles, joints, and even internal organs. When activated, they send signals through the spinal cord to the brainstem and higher brain centers.
Nociceptive signaling can be broken down into three primary processes. The first is transduction, or the conversion of a noxious stimulus—such as a toe stub or a beesting—into an electrical signal. The second is transmission, which involves the relay of electrical signals from nociceptors in the peripheral nervous system to the central nervous system—in particular, the spinal cord and brain. The third is modulation, whereby neurons within the spinal cord can either amplify or dampen the signal. After all this comes a step that is no longer nociceptive signaling but instead the brain’s interpretation of the nociceptive signals. This last step may or may not result in the experience of pain.
