Islāmic psychotherapy is more than a set of techniques; it is a deeply relational practice that interconnects ethical, cultural, and spiritual dimensions with the science of mental health. For Muslim clients, therapy is not only about alleviating psychological distress but also about attending to spiritual, moral, and cultural dimensions of life. Ethical dilemmas in Islāmic psychotherapy emerge from the complex intersection of religious, cultural, and psychological principles, presenting challenges that demand careful reflection and thoughtful resolution. Unlike conventional psychotherapy, which primarily operates within secular frameworks, Islāmic psychotherapy integrates spiritual and theological insights into the therapeutic process. While this integration offers clients profound opportunities for personal growth and alignment with faith, it simultaneously raises critical ethical questions: How can therapists respect the sacred while adhering to professional codes of practice? How can they navigate dilemmas around identity, morality, and cultural expectations without imposing personal beliefs or compromising client autonomy? How can universal ethical frameworks, often grounded in Western traditions, be reconciled with the rich moral and spiritual heritage of Islām? These questions are not theoretical, they arise daily in clinical practice, in the lived experiences of clients and the decisions therapists must navigate. The purpose of this work is multifold. It is primarily focused on the ethical dilemmas and challenges inherent in Islāmic psychotherapy. The opening chapters provide context, offering an overview of Islāmic psychotherapy, its epistemological foundations, and its ethical framework. Ethical behaviour remains a foundational pillar of this work, positioning Islāmic psychotherapy as a coherent and ethically grounded paradigm rather than a loosely affiliated set of practices. The framework articulated in this book emphasises the integrative relationship between spiritual, moral, and psychological dimensions of human experience, recognising ethics as central to both therapeutic intent and clinical practice. Within this paradigm, ethical decision-making is understood as a dynamic process informed by classical Islāmic source, such as Qur’ânic principles, prophetic guidance, and scholarly tradition, while also engaging rigorously with contemporary professional and regulatory standards. This integrated approach provides a robust foundation for clinicians to navigate complex therapeutic contexts with integrity, cultural attunement, and professional accountability. Islāmic psychotherapists navigate unique ethical challenges, including respecting client autonomy and informed consent in contexts where family and community expectations exert significant influence. Boundaries and multiple roles present further dilemmas. Practitioners may embody roles such as tarbiyah (educating), irshād (guiding), tazkiya (purifying), da‘wah (preaching), taṭbīb (healing), shifa (curing), wisāṭah (mediation), istiṭā‘a (empowering), ra‘ya (caretaking), and nasīha (advising). While these roles support holistic spiritual, emotional, and social well-being, their integration within conventional psychotherapy can create tensions, including risks to confidentiality, role confusion, and potential conflicts of interest. Therapists must also navigate issues of competence, power, and collaboration. Without sufficient theological literacy, addressing clients’ faithbased struggles can inadvertently cause harm, while overstepping into the role of religious authority can compromise professional boundaries and autonomy. Collaboration with community leaders or Imāms, though valuable, requires clearly defined roles to prevent contradictory guidance or undue influence. Additional challenges arise in reconciling Western-derived ethical frameworks with Islāmic moral principles. Mainstream professional codes do not always address issues salient in Muslim contexts, such as gender roles, sexuality, or religious guilt. Therapists must therefore balance professional standards with Islāmic ethical guidance, ensuring therapy remains spiritually sensitive, culturally responsive, and ethically coherent. Addressing these dilemmas is essential for safeguarding client well-being, maintaining therapeutic integrity, and fostering a practice that is both professionally responsible and spiritually grounded. This book explores these issues in depth. Chaps. 1 through 3 provide theoretical and ethical foundations, examining the intersections of psychotherapy, spirituality, religion, and ethics, as well as the historical contributions of classical Islāmic scholarship. Chaps. 4 through 9 examine ethical dilemmas, boundary challenges, client diversity, and working with vulnerable populations, including survivors of trauma, refugees, and marginalised groups. Chaps. 10 through 12 address contemporary and applied challenges, including digital therapy, structured ethical decision-making, and clinical supervision. Chap. 13, is especially significant for its provision of a comprehensive, practice-oriented ethical framework that enables Muslim psychologists to address professional obligations, ethical decision-making, and culturally grounded care within diverse clinical and institutional contexts. The final chapter, Reflections and Future Directions, synthesises insights, identifies gaps, and outlines pathways for advancing Islāmic psychotherapy as an ethically and spiritually informed field.
