Pediatric patients are a specialized patient population. Prescribing pediatric dosing needs individualized calculations for most medications based on patient age, weight and sometimes body surface area which increase the potential for errors to this patient population as a result of calculation inaccuracies.  The pediatric medication booklet was developed for the purpose of standardizing medication dosing, preparation and administration methods to reduce the need for dosing calculations and the risk of medication errors.  This booklet provides pre-calculated dose values (of the commonly used intravenous and oral medications in the pediatric intensive care unit at National Heart Institute) based on patient age and weight and consistent with the preparation method of each medication.  The preparation methods of intravenous medications are selected to meet the fluid restriction needs of post cardiac surgery pediatric patients as we used the minimum dilution volumes required for preparation and administration.  For small patients: we sometimes use a more acceptable dilution volumes rather than minimum volumes to facilitate preparation and administration process.  The medications in this booklet are presented in the form of tables. Each table includes three parts:  The first part: medication conc. available at the hospital and medication dosing  The second part: preparation and administration method, chemical/physical stability of medication after reconstitution/ dilution, compatibility and storage considerations  The third part: weight based dosing charts consistent with the preparation and administration methods used for each medication
