Regenerative medicine has undergone a profound conceptual transformation over the past two decades. Initially driven by the promise of direct cell replacement, the field has increasingly shifted toward understanding cells as dynamic biological factories capable of modulating tissue repair, immune responses, and microenvironmental homeostasis. Among the various cellular platforms explored, mesenchymal stromal/stem cells (MSCs) have emerged as one of the most clinically advanced and biologically versatile systems. More recently, their secreted extracellular vesicles (EVs) have gained significant attention as a cell-free therapeutic alternative, offering unique advantages in safety, scalability, and regulatory feasibility. This book was conceived to provide a comprehensive and translationally oriented synthesis of MSC and MSC-derived EV biology, bridging fundamental mechanisms with clinical application and industrial development. While numerous reviews and articles exist in this rapidly evolving field, there remains a critical need for an integrated resource that connects molecular biology, experimental evidence, manufacturing considerations, regulatory frameworks, and future therapeutic strategies within a single coherent narrative. The primary aim of this book is to serve as a roadmap for researchers, clinicians, and industry professionals navigating the transition from experimental discovery to clinical implementation. Each chapter is structured to progressively build understanding— from stem cell fundamentals and MSC biology to EV biogenesis, functional mechanisms, delivery strategies, preclinical evidence, and ultimately regulatory and commercialization pathways. Particular emphasis has been placed on methodological rigor, reproducibility standards, and the emerging concept of EVs as programmable biologics. The intended audience includes graduate students, biomedical researchers, clinicians in regenerative medicine, and professionals in biotechnology and pharmaceutical development. By combining mechanistic insights with translational perspectives, this book seeks not only to summarize the current state of the field but also to highlight unresolved challenges and define priorities for future research. We hope this work contributes to advancing the scientific and clinical development of MSC- and EV-based therapies and supports the realization of precision regenerative medicine.
