The role of the gastroenterologist continues to evolve. With the development and widespread use of therapeutic and interventional endoscopy, gastroenterologists are being called upon with increasing frequency to care for patients with known or suspected cancer. Patients requiring tissue acquisition, biliary decompression, relief from luminal obstruction, cancer screening, and post-treatment surveillance are now commonly encountered, and modern gastroenterologists need to be equipped with the knowledge and technical expertise to manage these and other cancer-related clinical problems. In the 49 chapters that follow, some of the most challenging and complex questions in gastrointestinal cancer care are explored in detail with an eye on useful, practical advice for clinical gastroenterologists. Key issues in the diagnosis and management of clinical problems associated with esophageal, gastric, and small and large bowel cancers are covered. Extensive space in this book is also devoted to pancreatic, biliary, and primary hepatic tumors as the rapid rise of endoscopic ultrasound (EUS) and the ongoing widespread use of endoscopic retrograde cholangiopancreatography (ERCP) have made gastroenterologists the “point of the spear” for both diagnosis and management of tumors that were in the past primarily cared for by surgeons. Additional chapters focus on important problems in the diagnosis and management of ampullary cancers, gastrointestinal stromal tumors (GISTs), hereditary cancer syndromes, screening and surveillance of colorectal cancer, anal cancer, and other relevant issues. This book strongly endorses the multidisciplinary approach to the care of patients with gastrointestinal cancers that is employed at most major cancer centers. In addition to the chapters written by gastroenterologists, many of the individual chapters are written by general surgeons, medical and surgical oncologists, and interventional radiologists. These authors provide valuable perspective on how interlocking care from multiple physicians can result in the best outcomes for cancer patients. The chapters in this book also frequently focus on complications—both how to avoid them and how to manage them when they arise, as, despite our best efforts, not every patient has an ideal outcome. An awareness of potential complications is the first step in their avoidance, and the risks of the techniques and technologies discussed in this book are emphasized for the reader to assist in making the best clinical choices for patients who are often acutely ill in the setting of a chronic, debilitating illness. I am indebted to my many coauthors for their contributions; without their efforts, this project would not have been possible. My personal passion for, and experience with, the diagnosis and management of gastrointestinal cancers spurred me to create this book.
