The pace of scientific advance, the huge expansion in the number of scientific journals and scientific meetings, together with the range of different voices on social media mean that new information about heart failure is now rapidly disseminated. There’s a darker side to the information boom, too, in the shape of predatory journals with few editorial controls and a weak peer review process, and widespread uninformed discussion in unmediated web fora. It is inevitable that when any book is first published, it will be at the least several months out of date due to the time lags inherent in the process. An example is that during the preparation of this second edition of the Oxford Textbook of Heart Failure, the SGLT2 inhibitors have gone from being possible add on therapies in the management of type II diabetes to being one of the (current) pillars of heart failure management. So what, then, is the role of the multi-author text book? New information and knowledge has to be assimilated and placed together with what is already known before it can be used to inform change, and that is particularly true of medical practice when ill-applied data can have catastrophic consequences. We are privileged to have colleagues joining us in this project who have an international reputation for their contribution to the scientific understanding of heart failure, but who are also superb communicators. They have the necessary ability to distil new developments and put them into context. They have a profound understanding of the subject and have the experience to distinguish the wheat from the chaff, what is important from the ephemeral. We hope this expanded second edition, with its text thoroughly revised and rewritten and with more than 10 new chapters on the aetiology, investigation and management of heart failure, provides a definitive reference volume for all those interested in managing patients with heart failure. We would like to thank the contributors to the book: without their efforts the book would not have been possible; our mentors, whose unflagging support and example led us to become specialists in the most exciting medical specialty; and our patients, for whose benefit the book, we hope, is written.
