This book is intended to provide an in-depth and up-to-date information of the various aspects of the association between blood pressure disorders and acute/chronic kidney diseases. In addition, our purpose is to discuss the pathophysiologic mechanisms underlying blood pressure elevation in renal disorders, the cardiorenal relationships and the interaction between the kidney and the extrarenal mechanisms controlling circulation, and the therapeutic approaches in hypertensive nephropathies. The book also includes hypertensive nephropathies infrequently recognized by clinicians, and its spectrum includes glomerulonephritis, nephrolithiasis, polycystic kidney disease, renal artery stenosis, reninomas, drug-related kidney injury, and rarely recognized causes of a blood pressure elevation such as renal embolic disease and blunt renal trauma. Treatment focuses on classical but also new antihypertensive drugs, the very frequently needed drug combinations but also on renal replacement therapy, other surgical approaches, and renal denervation. Kidney disorders in dismetabolic conditions such as diabetes and pregnancy are separately considered. Co-existence of hypertension and kidney disease is very common. Renal parenchymal diseases represent the most frequent form of secondary hypertensiovn, accounting for over 5% of cases referred to a hypertension clinic. Underlying renal pathology and severity of renal functional impairment are crucial factors in the determination of the degree of blood pressure elevation, with frequently higher blood pressure values in glomerular than in tubulointerstitial nephropathies. In renal disorders, the blood pressure elevation may be so severe as to lead to the clinical picture of malignant hypertension as well as to account for the largest percentage of patients with resistant and refractory hypertension. This book is divided into a number of sections. Within each section, the chapters have been authored by well-known experts in the Nephrology and Hypertension areas.
